By signing this “VEHICLE CONDITION FORM,” I am accepting this vehicle in its present condition, having completed a “Visual Walk Around Inspection” of the “Interior & Exterior” of this vehicle. I have been shown how to operate “Headlights, Wipers, Horn and Emergency Break”. During the walk-around visual inspection of the vehicle, I checked with the rental agent present at the time that all “Seat Belts” were in good working order.  My signature is proof that I am accepting the vehicle in its present condition and I am responsible for any new damage or loss of the vehicle.
